Can be anyone, anywhere, but, I'm talking about characters that make decisions that make absolutely no sense other than, "If I'm not this stupid, the plot can't progress."
"Space Seed" Kirk: "Sure, take a look at detailed ship schematics, mysterious, deliberately condescending stranger."
TWOK: Kirk: "Sure, they've lied to me already and there's all kinds of hinky going on with the ship assigned to service The Genesis Project, that project that Dr. Marcus just called me in a panic about someone coming to take, but that's the Reliant out there. That's a Starfleet ship. There must be some logical explanation. Let's keep the shields down."
Skynet: "I want to win, but I also like tension and dramatic effect, so, no, I will not send three Terminators at one time to kill Sarah because that would be unfair."
DS9 "Past Prologue" Tahna Los: "Destroying the entrance to the wormhole will ensure Bajoran independence!" Why exactly? Kira is correct. The wormhole is literally the key to Bajoran independence. With it, Bajor is destined to be a singular hub of commerce and scientific research.
But, see, Tahna. The Federation was there BEFORE the wormhole and they were there BEFORE the wormhole because Bajor couldn't feed itself. So...................yay famine and shattered society? What's the endgame here? The Federation would just go back to its original mission.

Batman: By not killing the Joker because some stupid philosophy, he effectively killed every person the Joker killed after Batman had his first chance to kill him... Mario: He's oblivious that Peach Toadstool is getting kidnapped on purpose....

True. A lot of my friends joke that "Getting Kidnapped" is a front for Bowser and Peach's affair.
Any Monster from Power Rangers. They just stand there while the Power Ranger's Zords can do all their transforming and what not. to put it all together and beat the snot out of the monster. I mean, blast the hell our of them while their gears are exposed while transforming...better yet. They always know the True Identities of the Power Rangers. Just go to their houses and destroy them while they're asleep, its so simple!!!
Tarkin. Just blast the giant planet that is blocking the way to the rebel base. Trust me, just blow up the dern planet, and the moon will go with it. It'll be fun watching it get pulverized by asteroids.
The Entire Federation Council, kissing Cardassians boots until they join sides with the dominion and screw you over. Just think, just maybe, the Maquis were right. And after the Setnik 3 massacre, you should have pulverized the Cardassians back to their home world. You clearly had the means.

Star Trek, due to its long history has numerous exemples. I'll have to think about this a bit more.
That said, I've always liked when it was adddressed in-universe as in this interesting exemple from the Firefly Movie Serenity:

VGR: "Instead of leaving a foolproof remote detonator behind to destroy the Caretaker array after we've used it to get home, let's stay in the Delta Quadrant, because we're responsible for the Ocampa for some reason (...?)." The simplest device, a remote-triggered bomb (something used in TOS' The Doomsday Machine), could've prevented the Voyager's stranding from ever happening.

Pretty much every character making any decision in the film Prometheus.... Any time they didn't know what to do with the characters in The Walking Dead... Which was basically always. It was the same cycle over and over: find a place, be okay for a while, then be driven out. Worse, the place usually was never just their place, it was some other community they happened to come by, so they destroy them, too.

Goku...ugh....many many times. He gives ruthless killers like 10 chances to live and what happens is either an ally finishes off the villian, or there are bad repercussions in the future.
